Remote Patient Service Coordinator

100% Remote | Healthcare Call Center

Join a growing healthcare team helping patients transition home after hospital discharge.

What You'll Do
  • Make outbound calls to recently discharged patients to schedule Nurse Coach assessments
  • Answer inbound calls and assist patients with scheduling
  • Coordinate care with patients, physicians, and nurse coaches
  • Document all patient interactions accurately
  • Follow scripted outreach and engagement processes
  • Complete 50โ€“100 calls daily while meeting quality and productivity goals
Requirements
  • 1+ year of call center experience (required)
  • High-volume customer service, outreach, or patient support experience
  • Strong communication and documentation skills
  • Ability to work an 8-hour shift in a remote call center environment
  • Weekend availability required
  • High school diploma or equivalent
  • Reliable internet and private workspace
