Key Responsibilities

As a Licensed Professional Counselor, your day-to-day responsibilities will include:

  • Conducting ongoing assessments tailored to the specific needs of each patient, ensuring personalized care plans are developed and monitored.
  • Engaging in consistent communication with supervisors, family members, and other relevant parties regarding progress and changes in each patient's treatment plan.
  • Creating and providing a tentative schedule of appointments, home visits, and client observations, ensuring organized and timely care delivery.
  • Coordinating home visits and in-home observations to collect important data that will inform the development of the patient's treatment plan.
  • Ensuring the effective implementation of individualized program plans, actively monitoring progress and adjusting as needed.
  • Participating in interdisciplinary team meetings and contributing valuable insights to care discussions.
  • Documenting monthly and quarterly progress notes to keep comprehensive records of all client interactions, progress, and outcomes.
  • Providing training and resources to staff to ensure that all team members are proficient in implementing the program plans and maintaining accurate documentation.
  • Offering follow-up interviews, home visits, and additional support to clients, reinforcing their care plans as necessary.
  • Acting as a liaison between the clinic and the broader mental health community, advocating for our patients and fostering relationships that can enhance care delivery.
Patient Population / Client Focus

The Licensed Professional Counselor at Reliant Health Center will work with a diverse population comprising both adults and youth, addressing an array of mental health challenges including, but not limited to, anxiety, depression, trauma, and substance misuse. Our holistic approach recognizes the importance of tailored interventions to meet the unique needs of each client, fostering resilience and empowerment.

Qualifications

To be considered for this role, candidates must possess:

  • A Master's degree or Doctoral degree in Counseling, Psychology, or a related field.
