Key Responsibilities
  • Conduct thorough and ongoing assessments to tailor counseling techniques and interventions to meet the specific needs of each patient.
  • Communicate effectively with supervisors, direct care staff, family members, and other collateral stakeholders regarding patient progress and treatment adjustments.
  • Create and maintain tentative schedules for sessions, home visits, and observations to monitor patient advancements while ensuring optimal use of time and resources.
  • Coordinate and execute home visits to conduct in-home observations, chart reviews, and staff interviews, enabling comprehensive data analysis to inform treatment plans.
  • Ensure that treatment plans are effectively implemented and monitored for consistency and effectiveness as per clinical guidelines.
  • Actively participate in Interdisciplinary Team Meetings and contribute to discussions that enhance collaborative care strategies.
  • Complete timely and accurate monthly and quarterly progress notes that document patient achievements and areas needing improvement.
  • Initiate and facilitate training sessions to equip staff and team members with the skills necessary for implementing programs and maintaining proper documentation.
  • Provide ongoing support through follow-up interviews, home visits, and observations to adapt approaches to patient needs.
  • Maintain meticulous records to ensure transparency and accessibility of information for all involved parties.
  • Serve as a liaison between the clinic and the community, advocating for mental health resources and supporting community development initiatives.
  • Assist in creating effective procedures and training materials that enhance program quality and staff competency.
