• Conduct detailed psychosocial assessments to identify patients' emotional, social, and financial needs, determining the appropriate interventions and support required.
• Provide evidence-based individual and group counseling that addresses issues such as adjustment, grief, trauma, anxiety, and coping strategies for diverse patient populations.
• Coordinate care transitions by working closely with community organizations post-discharge, ensuring patients receive the necessary resources for continued support.
• Participate in regular interdisciplinary team meetings to collaborate on treatment plans that integrate both clinical and psychosocial interventions.
• Maintain accurate and timely clinical documentation in compliance with healthcare regulations and organizational policies.
• Offer crisis intervention and support, navigating emergency situations with dignity and compassion.
• Engage in continuous professional development through training programs and workshops.

This role focuses primarily on patients seeking behavioral health support in a variety of settings, from young to elderly individuals. Our clientele includes those experiencing mental health disorders, stress-related conditions, and individuals requiring ongoing psychosocial support. You will work with vulnerable populations, ensuring that patients and their families receive empathetic guidance throughout their healthcare journey.

• Master's degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Illinois.
• Strong understanding of mental health issues, psychosocial aspects of illness, and community resources.

The compensation for this position ranges from USD 70,000 to 100,000 annually, depending on your experience and expertise.
