This position is performed remotely, allowing for flexibility while ensuring you are consistently available to our clients through virtual consultations and follow-up sessions. • Conduct comprehensive assessments of seniors to identify their psychosocial needs, emphasizing emotional, mental, and social factors that affect their health and quality of life.
• Collaborate with clients and their families to design and implement personalized care plans that respect their unique goals and preferences.
• Provide counseling sessions to support seniors through life transitions such as retirement, loss of loved ones, or health challenges, utilizing empathetic listening and effective therapeutic techniques.
• Advocate for seniors’ rights by helping them access community resources, including programs and services designed for older adults, ensuring they receive the necessary support.
• Maintain accurate and up-to-date client records, developing reports that meet regulatory and clinical guidelines, and collaborate with multidisciplinary teams to ensure comprehensive care.

This role specifically focuses on the senior population, addressing their distinctive mental health needs. The ideal candidate will demonstrate a deep understanding of geriatric issues, including depression, anxiety, and the effects of chronic illness. You will engage directly with clients and their families to provide support tailored to life challenges commonly faced by older adults.

• A Master’s degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of practical experience in a social work position, with a strong preference for those who have worked in community settings or specifically with the elderly population.
• In-depth knowledge of geriatric care principles, social work ethics, and local community resources.
• Strong communication, assessment, and problem-solving skills tailored to effectively manage complex client situations.

• Experience in a community health clinic or a similar environment.
• Familiarity with telehealth practices and digital record-keeping systems.
• Skills in counseling techniques specific to older adult populations.
