RN Remote Patient Monitoring - Summit Medical Group

Summit Medical Group is recruiting for a Remote Patient Monitoring Registered Nurse position. This is a full time hybrid position that allows work from home, with minimum office hours quarterly upon successful completion of introductory period. The best candidate for this role has nursing experience in cardiac, pulmonary, acute care or critical care. This position plays a critical role in healthcare, utilizing technology to monitor patients' health remotely. This job typically involves observing and analyzing patient data, identifying potential health issues, and collaborating with other healthcare professionals to provide effective patient care. The role contributes to the clinical care of the patient through remote support.

Examples of Duties (List does not include all duties assigned)

  • Patient Monitoring and Assessment:
  • Follow up with patients with reported abnormal readings/clinical findings to gather more information on their status and triage response according to data available.
  • Utilizes interview/evaluation in assessing patients and applies nursing judgement, consistent with practice standards, in formulating and making recommendations to the client.
  • Implements appropriate tools and pathways/protocols and educates the patient on how to initiate medications/treatments as prescribed by the provider on set protocols.
  • Communicate effectively with patients to understand their symptoms and concerns
  • Care Coordination:
  • Collaborate with physicians, specialists, and other healthcare professionals to develop and adjust patient care plans based on remote monitoring data.
  • Provide timely updates to the healthcare team (through EMR messaging) regarding any significant changes in a patient's condition.
  • Facilitate and schedule appointments with providers as appropriate.
  • Patient Education and Support:
  • Educate patients and their families on managing their conditions and the proper use of remote monitoring equipment as well as self-awareness with the data.
  • Address any queries or concerns patients may have about their health or the monitoring process.
  • Stay current on health education and literacy for the major chronic conditions experienced in the Medicare population.
  • Documentation and Compliance:
  • Maintain accurate and detailed patient records, ensuring confidentiality and compliance with healthcare regulations.
  • Document all interactions and interventions made based on remote monitoring.
  • Emergency Response:
  • Recognize and respond to emergencies based on remote data, helping the patient and family to initiate emergency services as needed.
  • Update healthcare team of actions taken by patient if an urgent situation arises.

Additional duties include identifying SMG patients with the potential for high-risk complications and coordinating the appropriate treatment in conjunction with the patient and Care Coordination team. Develop and implement targeted strategies to improve overall patient health and quality of life outcomes. Coordinate with SMG patient's primary care provider, specialists, community resources, families, and care programs, to ensure comprehensive, holistic, person-centered approach to care. Provide patients with evidence-based education on chronic medical conditions. Conduct chart reviews, educate patient, and facilitate follow up with provider to promote completion of age-appropriate preventive care procedures. Establish an environment of open communication, using motivational interviewing techniques to enhance patient's motivation toward change. Establish collaborative relationships with referral sources, community resources, and care providers. Act as an advocate for an individual's health care needs by identifying and communicating potential needs to the patient's provider.

Maintain security and privacy of patients by keeping patient care information confidential. Participate in required training and attend team, departmental, and organizational meetings. Monitor and improve quality of services provided to patients/caregivers through ongoing participation in team and departmental quality improvement activities. Update job knowledge by participating in educational opportunities that support the advancement of Care Coordination. Participate in the orientation and training of new staff as needed. Maintains strictest confidentiality both internally and externally. Adheres to expectations of the established corporate compliance plan. Performs related work as assigned.

Education: High School Diploma or equivalent required. Associates or Bachelors in Nursing required.

Experience: At least six months nursing experience in cardiac, pulmonary, acute care or critical care preferred.

Certification/License: Active state of TN RN license required.
