Key Responsibilities

• Conduct comprehensive psychosocial assessments to identify patients' needs and challenges, ensuring a thorough understanding of their emotional, social, and environmental factors.
• Develop, implement, and manage individualized treatment plans focusing on therapeutic interventions and resource referrals to enhance patients' coping strategies and skills.
• Maintain accurate and timely documentation of patient interactions, progress, and treatment plans in compliance with organizational and regulatory standards.
• Formulate strategies to foster caregivers' responsive parenting skills, addressing family concerns and reinforcing positive interactions based on children's social and emotional needs.
• Review clinical data and collaborate with other healthcare professionals to ensure a holistic approach to patient care, advocating for community resources that benefit overall health outcomes.

Patient Population / Client Focus

The Clinical Social Worker will work extensively with a diverse client base, including children, adolescents, adults, and seniors facing various psychological challenges. Qualifications

Compensation and Benefits
