• Maintain accurate and timely records, documenting clinical and progress notes to align with department policy and ensure compliance with legal standards.
• Collaborate with a multidisciplinary healthcare team to develop comprehensive treatment plans, including necessary discharge plans grounded in detailed psychosocial assessments.
• Engage families and support systems through consultations regarding patient treatments and discharge planning, ensuring a unified approach to care.
• Create individualized treatment plans tailored to the unique needs and circumstances of each patient, focusing on sustainable recovery and well-being.
• Conduct regular counseling sessions with patients, adjusting treatment strategies based on ongoing evaluations of their progress and circumstances.
• Implement continuous assessment techniques to gauge patient improvements and adapt therapeutic approaches as necessary.
