Key Responsibilities
  • Act as a liaison between families, outpatient providers, and hospice staff to coordinate comprehensive care plans.
  • Engage with insurance companies for pre-certification and utilization management, ensuring seamless access to services for clients.
  • Participate actively in discharge and aftercare planning, offering guidance and support tailored to each client's needs.

Qualifications
  • A Master's degree in Social Work (MSW) from an accredited institution.
