Located in West Hempstead, New York, Empower Well Health Care Center is a community health clinic dedicated to providing high-quality outpatient behavioral health services. We specialize in a variety of therapeutic modalities including individual and group therapy, substance abuse assessments, and treatment programs. This role is structured for a remote setting, allowing for greater flexibility and work-life balance. • Conduct thorough assessments to identify patients' emotional and behavioral issues, providing appropriate diagnoses and treatment plans.
• Engage in crisis intervention and offer immediate support to patients experiencing acute mental health needs.
• Communicate regularly with a multidisciplinary healthcare team, including primary care physicians, to relay patient progress, treatment plans, and emerging concerns.
• Document all patient encounters meticulously, maintaining records in accordance with departmental policies and compliance requirements for clinical and progress notes.
• Collaborate with fellow healthcare professionals to develop and implement individualized treatment plans, including discharge strategies that consider psychosocial factors and evidence-based interventions.
• Actively participate in community outreach by establishing and nurturing relationships with key community leaders and organizations, addressing the social needs of our patients.

Our clinic serves a diverse population with varying mental health needs, including individuals with anxiety, depression, substance use disorders, and complex social situations. Counselors at Empower Well are trained to adapt therapeutic approaches to meet the specific cultural and contextual needs of each patient, enabling more effective and personalized care.

• A Master's degree in Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling is required.
• Must hold a valid independent license such as Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).
• Minimum of two years of professional counseling experience is required, particularly in clinical environments.

• Experience in community health clinics or outpatient settings is a plus.
• Familiarity with various therapeutic models and evidence-based practices will be advantageous.
• Skills in engaging with underserved populations and addressing systemic barriers to mental health care are preferred.
