• Deliver comprehensive assessments and personalized care plans, drawing on evidence-based practices to guide your therapeutic approach.
• Provide solution-focused therapy through telehealth sessions that respect client confidentiality and comfort.
• Connect clients with vital community resources to aid in their hospice or end-of-life journey, ensuring they have support systems in place.
• Collaborate with your remote team to share insights and strategies aimed at improving client outcomes, using case reviews and weekly meetings to stay aligned with best practices.
• Accurately document client interactions and progress notes in accordance with state regulations and organizational guidelines, ensuring continuity of care.

In this role, you will primarily work with patients and families dealing with end-of-life issues, which requires a sensitive and empathetic approach. Your expertise will aid these individuals in navigating complex emotions and the implications of terminal illnesses. Understanding their unique needs and providing tailored support throughout this critical stage of life will be central to your responsibilities.

• A Master of Social Work (MSW) degree from an accredited institution.
• A valid LCSW license in California, permitting you to practice as a licensed clinical social worker.
• At least two years of direct social work experience, preferably in hospice, palliative care, or related fields.
• Proficient in using telehealth platforms and various digital tools for communication and documentation.
• Strong interpersonal skills that foster trust and effective engagement with clients.
