• Conduct thorough assessments to evaluate clients' presenting issues, social history, and treatment objectives.
• Provide compassionate individual and family therapy using evidence-based modalities, such as cognitive-behavioral therapy (CBT), grief counseling, and mindfulness techniques.
• Develop and implement personalized treatment plans that align with the unique circumstances of hospice patients and their families.
• Engage in regular collaboration with interdisciplinary teams, including physicians, nurses, and spiritual care providers, to ensure comprehensive support for clients.
• Maintain adherence to ethical standards and legal regulations while preserving client confidentiality at all times.
• Participate in ongoing professional development to stay updated with the latest advancements in mental health practices and hospice care.
• Build trusting therapeutic relationships that foster healing, emotional resilience, and constructive communication.
• Offer practical coping strategies and emotional support to help clients navigate the challenges associated with end-of-life issues.

In this role, you will work closely with hospice patients who present a variety of emotional responses to end-of-life situations, including anxiety, depression, grief, and family conflict. Your expertise will not only aid in addressing the psychological needs of individuals facing terminal illnesses but also support their families as they cope with loss and transition.

• Master's degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Minimum of two years of clinical experience, particularly within a hospice or serious illness setting.
• Demonstrated ability to develop and implement effective treatment plans based on client needs.
• Strong interpersonal, communication, and empathy skills to engage effectively with clients and their families.

• Experience working in hospice care or a similar palliative care setting.
• Familiarity with grief counseling, supportive psychotherapy, and end-of-life issues.
• Proficiency in telehealth platforms and remote therapeutic modalities.
