• Conduct thorough assessments of patients' psychological needs and create individualized treatment plans that address their unique emotional and mental health challenges.
• Facilitate solution-focused therapeutic interventions tailored for patients suffering from trauma, anxiety, and depression.
• Maintain precise and confidential records of all patient interactions, assessments, and treatment plans, ensuring adherence to HIPAA regulations.
• Collaborate with a multidisciplinary team, including physicians, nurses, and other social workers, to coordinate holistic care that promotes well-being.
• Provide support and guidance to patients' families, addressing their concerns and helping them cope with the realities of terminal illness.
• Stay abreast of current best practices and emerging research in social work and hospice care to continuously improve service delivery.

In this position, you will work primarily with terminally ill patients and their families within a hospice environment. Our patient population includes individuals coping with end-of-life issues, requiring sensitive and empathetic care that helps them navigate their emotional and psychological journeys. You will also cater to diverse backgrounds and circumstances, necessitating a flexible and culturally competent approach to care.

• Minimum of a Bachelor's Degree in Social Work from an accredited institution.
• Active LCSW licensure in the state of Georgia.
• Proven experience in delivering social work services, particularly in hospice or palliative care settings.
• Strong communication skills, with the ability to connect with patients and families in often challenging situations.
• Ability to work well within a collaborative team, cultivating positive relationships with colleagues.

• Experience providing mental health support to patients experiencing trauma, anxiety, or depression.
• Previous work in a hospice, palliative care, or related field is highly desirable.
• Familiarity with electronic health records (EHR) systems and telehealth platforms.
