• Conduct thorough psychosocial assessments to identify the social, emotional, and environmental challenges faced by clients.
• Formulate and implement individualized service plans that detail specific goals and interventions, monitoring client progress over time.
• Deliver high-quality counseling sessions, utilizing evidence-based techniques to support individuals and families navigating crises and life transitions.
• Build trusting relationships with clients, helping them to develop strategies that empower them to access community resources and achieve their goals.
• Provide crisis intervention services, including risk assessments and safety planning, to ensure immediate support for clients in distress.
• Collaborate actively with local agencies and support organizations to coordinate services that holistically meet client needs.
• Participate in community outreach initiatives aimed at raising awareness of mental health issues and promoting the resources available at Health Access Network.
• Engage in team meetings, trainings, and case conferences to share knowledge and enhance service delivery.

You will primarily serve clients from various backgrounds facing significant life challenges, such as homelessness, domestic violence, substance abuse issues, and economic disenfranchisement. Your role will be crucial in offering support, guidance, and advocacy to help these individuals and families navigate their unique situations, fostering their resilience and self-sufficiency.

• A Master's degree in Social Work from an accredited institution.
• A current and valid state license as a Licensed Clinical Social Worker (LCSW).
• Strong foundation in trauma-informed care practices and a deep understanding of social and economic justice issues.
• Exceptional interpersonal, communication, and cultural competence skills, which will facilitate effective engagement with a diverse clientele.
• A proactive problem-solver with a collaborative spirit, able to work effectively in a remote setting.

• Experience working within a social service agency or a similar healthcare environment is strongly preferred.
• Familiarity with community resources, social service programs, and local healthcare systems in Nashville, TN is a plus.
• Demonstrated experience in developing and implementing individual service plans and facilitating access to needed resources.
