• Conduct comprehensive psychosocial assessments to identify and address clients' emotional, social, and financial needs.
• Provide individual and group counseling tailored to patients' specific challenges, including adjustment, grief, and coping mechanisms.
• Work closely with medical teams to develop holistic treatment plans that align with patients' healthcare journeys, ensuring a seamless integration of physical and mental health services.
• Coordinate with community organizations to arrange post-discharge services and facilitate ongoing support for patients transitioning out of care.
• Offer emotional support and educational resources to patients and their families regarding treatment options and available community resources.
• Participate actively in hospital committees and contribute your expertise in discussions on social work-related matters.
• Provide crisis intervention as necessary, ensuring that clients receive timely assistance during moments of acute distress.
• Document all client interactions, assessments, and treatment plans in compliance with organizational policies and legal standards.

As a Licensed Clinical Social Worker at Psychological & Vocational, you will support a diverse patient population characterized by a range of mental health conditions, including anxiety disorders, depression, trauma, and interpersonal challenges. Your approach will focus on individualized care, with an emphasis on understanding each client’s unique life situation and mental health journey, ensuring that every interaction is meaningful and impactful.

• A Master’s degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong understanding of psychosocial theory, professional ethics, and best practices in therapeutic interventions.

• A minimum of two years of experience in a healthcare or hospital environment is strongly preferred.
• Experience with electronic health record (EHR) systems and telehealth platforms is a plus.
• Familiarity with medical terminology and crisis intervention techniques is highly beneficial.
