• Conduct thorough assessments and screenings through virtual consultations to evaluate your clients’ mental health needs and devise tailored treatment plans.
• Develop individualized therapy plans, incorporating evidence-based practices to meet the unique psychological and emotional needs of hospice patients.
• Lead both individual and group therapy sessions, utilizing therapeutic techniques that promote healing and emotional resilience in patients and their families.
• Collaborate with an interdisciplinary team including nurses, social workers, and physicians to ensure an accurate and comprehensive approach to patient care.
• Educate and support patients’ families by providing insights into mental health challenges and encouraging their active involvement in the recovery journey.
• Maintain precise and confidential documentation of patient interactions, treatment plans, and progress in line with professional and legal standards.

Your primary focus will be on individuals receiving hospice care, along with their families, who are often dealing with complex emotions stemming from terminal illness. This demographic requires specialized understanding and sensitivity as they navigate significant life transitions. You’ll play a pivotal role in facilitating open conversations about grief, loss, and living meaningfully during limited timeframes.

• A current and valid license as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Doctor of Psychology (PsyD) is required.
• A Bachelor’s Degree in Social Work, Psychology, or Counseling; a Master’s Degree is highly preferred.
• A minimum of 2 years of direct experience in mental health counseling, preferably within hospice or palliative care settings.
• A solid foundation in evidence-based approaches for addressing a range of mental health disorders.
• Exceptional interpersonal skills that foster trust and rapport between you, your patients, and their families.
• Ability to work effectively in a remote environment, demonstrating strong organizational and self-management skills.

• Experience in bereavement counseling and providing emotional support during grief processes.
• Familiarity with hospice frameworks, palliative care models, and understanding of psychosocial needs of patients facing end-of-life issues.
• Proficiency in using telehealth platforms and electronic health record systems.

• Competitive salary in the range of USD 60,000 – 104,000 annually, commensurate with experience and credentials.
