Young Minds Counseling P.C is actively seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in Erie, Pennsylvania. Key Responsibilities
  • Provide direct clinical services to assigned clients, including individual, group, and family therapy, based on presenting issues.
  • Conduct thorough assessments and screenings utilizing evidence-based practices to identify and address mental health needs.
  • Develop and implement personalized integrated care plans in collaboration with multidisciplinary teams, ensuring access to comprehensive linked services.
  • Participate actively in crisis intervention efforts, working closely with clients, families, and educational staff to address urgent mental health concerns.
  • Monitor client progress and outcomes, adjusting treatment plans as necessary to meet changing needs.
  • Provide post-crisis support and ongoing evaluations to ensure continuity of care.
  • Complete timely documentation and reporting to maintain compliance with agency standards and facilitate effective communication with other service providers.
