The Home Health Field Registered Nurse (RN) plays a critical role in delivering high-quality, patient-centered care within the comfort of patients' homes. This position involves comprehensive assessment, planning, implementation, and evaluation of nursing care tailored to individual patient needs, often managing complex medical conditions. The RN collaborates closely with interdisciplinary teams, patients, and their families to promote health, prevent complications, and support recovery or chronic disease management. By providing education, emotional support, and clinical expertise, the RN empowers patients to achieve optimal health outcomes and maintain independence. Requirements

    Responsibilities
    • Complete comprehensive patient assessments and develop individualized care plans.
    • Perform skilled nursing visits and ongoing evaluations of patient status.
    • Apply admission and recertification criteria to determine appropriate levels of care and patient eligibility.
    • Educate patients and families on disease management, medications, nutrition, and self-care techniques.
    • Coordinate care with physicians, caregivers, and interdisciplinary team members.
    • Assess caregiver capabilities and identify patient and family support needs.
    • Implement preventative, rehabilitative, and infection-control measures.
    • Supervise and provide clinical direction to Home Health Aides and LPN/LVNs.
    • Maintain accurate and timely clinical documentation.
    • Monitor assigned cases for compliance with regulatory and payer requirements.
    • Communicate changes in patient condition and response to treatment with the care team.
    • Ensure continuity of care through effective coordination and patient advocacy.
  • Qualifications
    • Current Registered Nurse (RN) license.
    • Valid Driver's License and current auto insurance.
    • Current CPR Certification.
    • Minimum of 2 years of Home Health RN experience preferred.
    • HomeCare HomeBase (HCHB) experience preferred.
    • Strong assessment, communication, and patient education skills.
