Care Manager – Social Work (Remote)

Location: Remote (Orientation in the Bronx, NY)
Schedule: Monday – Friday | 8:00 AM – 5:00 PM (1-hour lunch)
Position Type: Temporary Assignment (LOA Coverage through September 28, 2026)

About the Opportunity

We are seeking an experienced Care Manager – Social Worker to oversee care management services and ensure high-quality, participant-centered care. In this role, you will use your clinical expertise to review assessments completed by field staff, coordinate interdisciplinary care, and develop comprehensive care plans that address the medical, behavioral, psychosocial, and community needs of participants.

Must Haves
  • Master's Degree in Social Work (MSW) from an accredited program
  • Current New York State Social Work license (LMSW or LCSW)
  • Minimum of 2 years of care management or case management experience

  • Strong knowledge of community resources and healthcare systems
  • Experience reviewing clinical assessments and care documentation
  • Excellent communication, organizational, and critical thinking skills
  • Proficiency with electronic medical records (EMR) and Microsoft Office
  • Ability to work independently in a remote environment while collaborating with an interdisciplinary team
Nice to Have
  • Experience working with PACE, managed care, or Medicaid populations
