Wellness Coordinator (LPN)

Location: Remote (Florida Residents Only)
Job Type: Full-Time

At The Palms Home Care, we're passionate about helping patients remain safe, healthy, and independent in the comfort of their homes. We are seeking an experienced Licensed Practical Nurse (LPN) with exceptional scheduling, coordination, and communication skills to support our growing Home Health and Elite Rehab divisions.

This is a fast-paced, highly collaborative role that serves as the central communication hub between patients, therapists, clinicians, and branch leadership. The ideal candidate thrives in a dynamic environment, has outstanding organizational skills, and can effectively manage multiple priorities while ensuring patient visits are scheduled efficiently and clinical teams remain informed.

What You'll Do
  • Coordinate and manage daily scheduling for our Home Health and Elite Rehab clinicians, ensuring timely patient visits and optimal clinician productivity.

  • Maintain consistent communication with therapists, nurses, clinical leadership, patients, and caregivers to ensure schedules remain accurate and up to date.

  • Proactively identify scheduling conflicts, staffing gaps, and last-minute changes, implementing solutions quickly to minimize disruptions in patient care.

  • Serve as the primary liaison between field clinicians, patients, and branch leadership regarding scheduling and visit coordination.

  • Monitor daily schedules and productivity reports, ensuring all visits are appropriately assigned and completed.

  • Conduct wellness and follow-up calls with patients as needed to support continuity of care and patient satisfaction.

  • Maintain accurate documentation within the agency's EMR system.

  • Support Quality Assurance (QA) and Performance Improvement (QAPI) initiatives.

  • Ensure compliance with Medicare, Medicaid, AHCA, and agency standards.

What We're Looking For
  • Active, unrestricted Florida Licensed Practical Nurse (LPN) license.

  • Minimum of 2 years of scheduling, clinical coordination, or home health operations experience.

  • Previous home health scheduling or staffing coordination experience strongly preferred.

  • Proven ability to manage complex clinician schedules in a fast-paced healthcare environment.

  • Outstanding communication, organizational, and problem-solving skills.

  • Ability to multitask, prioritize competing demands, and adapt quickly to changing schedules.

  • Experience with WellSky/Kinnser or similar EMR systems preferred.

  • Comfortable working independently in a fully remote environment.

  • Reliable high-speed internet and a private HIPAA-compliant home office.
