Home Infusion Nurse - Partner With Patients. Practice With Purpose.

As a Home Infusion Nurse, you'll provide oneonone, skilled clinical care to patients where they feel most comfortable - at home or in an ambulatory infusion suite. This role offers true autonomy, a flexible schedule, and the opportunity to build meaningful connections with patients and caregivers while delivering lifechanging therapies.

You'll have the support of a multidisciplinary care team, comprehensive training, and the resources you need to deliver safe, effective, and compassionate infusion care.

In This Role, You Will:

  • Learn more about our Home Infusion Pharmacy: Optum Infusion Pharmacy

  • Work approx 40 hours a week (Schedule will be Wednesday-Friday, 12pm-10pm, and 10 hour shift on Saturday)

  • Travel within a certain radius to deliver care in patient homes or designated sites

Primary Responsibilities:

  • Travel to patient homes and related sites using a personal or rental vehicle in accordance with company travel policies
  • Provide safe and compliant infusion services in accordance with Optum Infusion Pharmacy policies, protocols, and physician orders
  • Establish and maintain positive therapeutic relationships with patients and caregivers, including education on self-care and self-administration
  • Assess patient suitability for home infusion therapy, including home safety, caregiver support, and patient independence
  • Obtain and maintain accurate patient medical histories, clinical assessments, and all required documentation, including plans of care, visit notes, medication profiles, and discharge summaries
  • Deliver skilled nursing care across a range of patient ages and conditions based on physician orders and treatment plans
  • Establish, access, and maintain vascular devices, including peripheral IVs, PICC lines, tunneled catheters, and implanted ports
  • Administer medications and monitor patient responses, including managing infusion reactions, vital signs, side effects, and lab collection
  • Communicate patient status and therapy response promptly with physicians, pharmacists, supervisors, and other care team members
  • Assist in staff orientation and training, serving as a clinical educator and resource as requested

Required Qualifications:

  • Active and unrestricted Registered Nurse (RN) license in the state of Arizona
  • Current BLS/CPR certification or willingness to obtain prior to start
  • 1 years of RN experience in a clinical setting supporting infusion or IV therapy (e.g., ICU, Med Surg, ED, PACU)
  • Ability to work independently with strong clinical judgment and effective interdisciplinary collaboration skills
  • Valid driver's license, reliable transportation, and ability to cover an 80 mile service radius of the Phoenix West Valley while working 35-40 hours a week. 

 

Preferred Qualifications:

  • Home infusion experience
  • Familiarity with Microsoft Office and EMR platforms
  • Knowledge of Infusion Nursing Society (INS) standards
  • Understanding of HIPAA, OSHA, and home infusion accreditation guidelines
  • Proven solid communication, organization, critical thinking, and teaching skills

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $28.94 to $51.63 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
