Position Overview:
We are looking for a Product Manager to join our Product team to own and grow a data domain spanning the full post-acute and value-based care continuum. You, as the Product Manager, will play a critical role in turning complex clinical and claims data into insights that help providers improve transitions of care, reduce readmissions, and deliver better outcomes across settings. You'll partner closely with data science, engineering, and GTM teams to ship AI-powered analytics that give post-acute providers and value-based organizations a clearer view of performance, referral patterns, and network leakage. This is a high-impact opportunity for someone who wants to shape how AI and clinical data come together to solve real problems in post-acute and value-based care, at a company positioned at the center of that shift.
Location: Our strong preference is for candidates to be within commuting distance of Atlanta or Philadelphia. We have team members in both locations who visit our office weekly on Wednesdays. However, we will also consider applicants in other U.S. locations in a remote capacity.
Reports to: Director, Product
As a Product Manager at Trella, you will:
  • Own product strategy, roadmap, and execution for a clinical data domain spanning the full care continuum
  • Lead end-to-end product development, from discovery through delivery, across cross-functional teams including data science, data engineering, and software engineering
  • Translate clinical and claims data into actionable analytics, including transitions of care analysis, patient outcomes, performance benchmarking, referral patterns, cost, and network leakage
  • Identify and integrate AI/ML-driven capabilities (e.g., predictive analytics, risk stratification, anomaly detection) to enhance clinical insights and drive smarter decision-making for value-based care and post-acute providers
  • Build and ship AI-powered product features, from concept through adoption, including generative AI, predictive models, and intelligent automation embedded directly into clinical and operational workflows for users
  • Use AI tools daily across the full product lifecycle, including discovery, prototyping, requirements definition, stakeholder communication, and GTM enablement
  • Drive measurable business outcomes including ARR growth, customer retention, and NPS, ensuring product investments deliver impact
  • Own the clinical narrative around care transitions, including discharge planning, referral patterns, and cross-setting patient flow, translating those workflows into product requirements
  • Independently prioritize and manage the product backlog, balancing clinical user needs, business value, and technical investment
  • Collaborate with GTM teams (Sales, Marketing, Customer Success) to support positioning, enablement, and successful product launches
  • Conduct market and competitive analysis to inform product direction and identify opportunities for clinical and AI-driven innovation
  • Champion a product-led mindset, driving clarity, accountability, and continuous improvement across teams

This job might be a fit for you if you have:
  • 4+ years of product management experience in B2B SaaS, with direct ownership of roadmap and product outcomes
  • Hands-on experience with clinical data sets across care settings (e.g., MDS, OASIS, ADT feeds, claims, EHR data)
  • Strong understanding of transitions of care, including how patients move across acute, post-acute, and community settings, and the data and workflows that support those handoffs
  • Deep familiarity with post-acute care workflows and performance metrics (e.g., SNFs, home health, hospice, referrals, readmissions, LOS)
  • Experience with or exposure to value-based care models (e.g., ACOs, bundled payments, network optimization)
  • Strong experience in healthcare analytics, with a preference for post-acute and/or value-based care domains
  • Proven ability to drive KPIs including retention, NPS, and revenue growth
  • Experience building or managing data products and analytics/visualization tools
  • Strong analytical and problem-solving skills, with comfort working closely with data science and engineering teams
  • Demonstrated ability to lead cross-functional teams and influence stakeholders including executive leadership
  • Experience in Agile environments and familiarity with tools like Jira, Aha!
  • A customer-first mindset with a passion for solving complex healthcare problems and improving patient outcomes
  • Strong understanding of CMS reimbursement and VBC models
  • Genuine enthusiasm for AI and a track record of using AI tools to do better, faster work
  • Clinician or prior clinical role with direct patient care experience across settings
