As a remote position within the Community Health Clinic setting, you will collaborate with a team of healthcare professionals to provide comprehensive care and support to families. Key Responsibilities
  • Conduct thorough evaluations and assessments for children and families to identify needs and developmental milestones.
  • Engage in the development and implementation of Individualized Family Service Plans (IFSP), ensuring that family voices are prioritized in the process.
  • Regularly monitor and evaluate outcomes to ensure effective service delivery and team collaboration.
  • Provide emotional support and educational resources to families aimed at promoting healthy child development.
  • Utilize evidence-based practices to create joint plans with families, focusing on Natural Learning Environment Practices.
  • Implement IFSPs based on the developmental status of children and family priorities, adjusting the frequency of services accordingly.
  • Employ coaching models and natural teaching techniques to foster growth in children within their everyday environments.
  • Adhere to federal and state regulations, maintaining the highest standards for health and safety within our services.
Patient Population / Client Focus

Your focus will primarily be on infants and toddlers experiencing developmental delays or disabilities. Through our Children’s Services program, you will have the opportunity to help young children acquire essential skills in various areas, including physical, cognitive, social/emotional, and communication development. Working closely with families, you will play a critical role in their children's growth during these formative years.

Qualifications
  • A Master's Degree or Doctorate in Social Work, with a valid LCSW license from the Department of Public Health.
  • A valid driver's license is required to facilitate travel to family homes and community events.
  • Demonstrated ability to adapt interventions to meet diverse learning styles and individualized treatment plans for children.
  • Strong organizational and time management skills to effectively juggle multiple families and evaluations.
  • Proficient in communication, with the ability to build rapport with children, families, and team members.
Preferred Experience
  • A minimum of one year experience in early intervention services with children and families.
  • Familiarity with Natural Learning Environment Practices is highly desired.

Compensation and Benefits
