This role is designed for a remote work setting, allowing you to operate effectively from your chosen location while ensuring a work-life balance. As part of a hospice center that specializes in family-centered care, you will have the flexibility to manage your time while collaborating with families and interdisciplinary teams to deliver exceptional early intervention services.

• Conduct thorough evaluations and assessments to identify the needs and strengths of children with disabilities or delays.
• Collaborate with families to develop and implement Individualized Family Service Plans (IFSPs) that align with the children's developmental goals.
• Regularly monitor and evaluate the effectiveness of interventions, maintaining open communication with healthcare professionals and family members.
• Foster strong relationships with children and families to create a supportive environment that encourages active participation in treatment.
• Design individualized treatment strategies based on evidence-based practices and the Natural Learning Environment Principles.
• Educate families on effective strategies to support their child’s development, enhancing their abilities to engage in learning and growth.
• Adhere strictly to all Federal and State regulations pertaining to early intervention services.
• Embrace coaching models and natural environment teaching approaches to ensure practical application of learned skills.

In this role, you will primarily work with infants and toddlers facing developmental challenges and disabilities. Your expertise will guide families through the complexities of early intervention, ensuring they receive the resources and support necessary for optimal child development. This focused approach not only champions the child's growth but also empowers families through education and skill-building.

• A Bachelor’s degree in Social Work or a related field; Master’s Degree or Doctorate preferred.
• Current and valid LCSW licensure from the Department of Public Health.
• A valid driver's license and reliable transportation for potential home visits.
• Minimum of one year of relevant experience in early intervention services with children.
• Strong understanding and commitment to Natural Learning Environment Practices.
• Excellent organization, time management, and clear communication skills.
• A collaborative mindset, valuing teamwork and diversity in professional settings.

• Bilingual capabilities are highly desirable to better serve diverse populations.
• Experience with coaching models and evidence-based intervention techniques is advantageous.
• Understanding of local community resources and support networks for families with children in need.
