Key Responsibilities

As an LCSW within our organization, you will:

• Provide psychological assessments and develop individualized treatment plans that align with evidence-based and solution-focused therapeutic approaches.
• Utilize a range of therapeutic methodologies, including Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and mindfulness techniques tailored to the unique needs of your patients.
• Maintain meticulous and timely clinical documentation in compliance with HIPAA and state regulations, ensuring high standards of patient care.
• Collaborate actively with a multidisciplinary team, including physicians, psychologists, and nurses, to create a holistic approach to patient treatment and support.
• Participate in case consultations and peer review meetings, contributing your insights to enhance the overall quality of care.
• Educate patients and families on mental health challenges, available treatment options, and community resources, empowering them to actively participate in their care plan.
• Stay updated on the latest research and techniques in clinical social work, integrating your findings into daily practice to benefit our clients.
• Manage a full caseload, ensuring accountability for treatment outcomes and adhering to ethical guidelines set forth by the National Association of Social Workers (NASW).

Patient Population / Client Focus

Your focus will be on individuals and families dealing with complex emotional and psychosocial issues related to serious illnesses. In this role, you will address not only the psychological aspects of their health challenges but also the social determinants affecting their care. Qualifications

• A Master’s degree in Social Work from an accredited institution.
• A valid LCSW license in the state of Illinois.
• A minimum of 3 years of clinical experience with demonstrable expertise in providing therapeutic services.

• Excellent interpersonal, verbal, and written communication skills.

Preferred Experience

• Experience in hospice or palliative care settings.

Compensation and Benefits
