• Carry out comprehensive psychosocial assessments via telehealth to accurately evaluate clients’ emotional, social, and psychological needs, identifying their strengths and areas for growth.
• Implement evidence-based therapeutic interventions designed specifically for hospice clients, using modalities that may include cognitive-behavioral therapy (CBT), mindfulness techniques, or supportive therapy.
• Collaborate with physicians, nurses, and other healthcare providers to coordinate integrated care plans that encompass physical and emotional support for hospice clients.
• Advocate relentlessly for clients’ rights, ensuring they have access to necessary community resources, enhancing their quality of life during their terminal stages.
• Maintain meticulous documentation of client interactions, treatment progress, and outcomes in electronic health records, ensuring compliance with regulatory standards and facilitating continuity of care.
• Engage in regular team meetings, participate in case conferences, and attend continuing education activities to enhance your clinical skills and stay informed about best practices.

• A Master’s degree in Social Work from an accredited program.
• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in Washington state.
• A minimum of two (2) years post-licensure experience providing clinical social work services, preferably in a hospice or palliative care setup.
• Demonstrated proficiency in conducting diagnostic assessments and formulating evidence-based treatment plans tailored to diverse client needs.
• Exceptional interpersonal and communication skills, with an emphasis on building rapport with clients from various backgrounds and experiences.
• A strong commitment to cultural competence, social justice, and ethical practice in all aspects of social work.

• Experience in hospice or end-of-life counseling, particularly with grief and loss issues.
• Proficiency in evidence-based practices such as Dialectical Behavior Therapy (DBT) or Acceptance and Commitment Therapy (ACT).
• Strong familiarity with electronic health record (EHR) systems and telehealth technology.
