Care Navigator (Remote)

Location: 100% Remote 

Schedule: Monday-Friday, 11:00 AM-7:30 PM EST

Pay Rate: $20/hour

Contract Length: 6 Months to potential hire

Start Date: September 14th 2026

Overview

TEKsystems is partnering with a healthcare client to hire Care Navigators who are passionate about helping patients access the care they need. In this fully remote position, you will serve as a critical link between patients and healthcare providers by coordinating referrals, scheduling appointments, and removing barriers to care.

This role is ideal for individuals with healthcare administrative, patient support, call center, or customer service experience who thrive in a fast-paced environment and enjoy making a positive impact on patient outcomes.

Key Responsibilities

Patient Engagement & Advocacy

  • Conduct outbound outreach via phone, text, and email to assist patients with scheduling appointments.
  • Educate patients on referral processes, next steps, and available resources.
  • Provide compassionate and culturally responsive support to improve patient engagement and reduce no-shows.
  • Act as a patient advocate by identifying and resolving barriers to care.

Referral Coordination

  • Manage and prioritize referral requests to ensure service level agreements (SLAs) are met.
  • Communicate with specialty providers and healthcare partners regarding referral status.
  • Track referrals through completion and ensure successful referral loop closure.
  • Escalate issues or delays as needed to support timely patient access to care.

Documentation & Compliance

  • Accurately document all patient interactions within EMR and CRM systems.
  • Maintain detailed and audit-ready records using standardized documentation practices.
  • Follow HIPAA guidelines and privacy regulations at all times.
  • Ensure data accuracy and completeness across multiple systems.

Operational Excellence

  • Manage personal work queues and dashboards to meet productivity goals.
  • Monitor referral volumes and proactively address outstanding tasks.
  • Adhere to quality assurance standards and internal workflows.
  • Support continuous improvement initiatives and operational best practices.

Required Qualifications

  • High school diploma or equivalent required.
  • Minimum of 2-3 years of healthcare administrative, patient support, medical office, care coordination, or related experience.
  • Strong customer service and communication skills.
  • Proficiency with Microsoft Office applications.
  • Experience handling administrative and clerical responsibilities.
  • Ability to multitask and manage competing priorities in a remote environment.
  • Strong attention to detail and organizational skills.

Preferred Experience

  • Healthcare call center experience.
  • Patient scheduling and referral management.
  • Experience working within EMR/EHR systems.
  • Knowledge of HIPAA compliance and healthcare documentation requirements.
