Position Summary

The Corporate Admissions Coordinator will be responsible for reviewing, coordinating, and processing skilled nursing referrals for a designated region of 6–7 Skilled Nursing Facilities (SNFs). Working from a centralized office or remote, this individual will serve as a key liaison between hospitals, case managers, referral sources, and facility teams to ensure timely and appropriate admissions decisions.

We are looking for someone with significant SNF referral management experience who can autonomously review and accept referrals with minimal oversight while maintaining strong census growth and payer mix optimization.

Experience overseeing admissions for multiple facilities or large-scale operations is strongly preferred.

Key Responsibilities

  • Review and process incoming referrals for a region of 6–7 SNF facilities
  • Evaluate clinical documentation, payer sources, and admission criteria to determine appropriateness for placement
  • Make timely admission decisions and communicate updates to referral sources
  • Coordinate with hospitals, case managers, insurance providers, and internal facility leadership
  • Manage referral pipelines to maximize census growth and occupancy across assigned facilities
  • Ensure accurate documentation and follow-up within admissions systems
  • Build and maintain strong relationships with referral partners and healthcare networks
  • Prioritize urgent referrals and manage multiple workflows simultaneously in a fast-paced environment
  • Collaborate with facility administrators and admissions teams to support strategic growth initiatives

Qualifications

  • Required: Strong experience in SNF admissions and referral management
  • Proven ability to independently review and accept referrals
  • Deep understanding of:

  • Insurance verification and authorization processes
