• Directly assess and address the needs of clients, conducting individual, family, and group therapy sessions designed to support their mental health and emotional stability.
• Collaborate with multidisciplinary teams to develop and implement integrated care plans that ensure seamless access to necessary services.
• Perform evidence-based screenings and assessments, promptly identifying symptoms and facilitating referrals for comprehensive evaluations when necessary.
• Maintain a caseload of clients, monitoring progress and modifying treatment approaches to align with each client’s evolving needs.
• Participate actively in team meetings and committees aimed at improving the quality of service delivery and organizational effectiveness.
• Provide crisis intervention as required, ensuring safety and appropriate responses to clients in distress.
• Complete administrative duties including documentation of services, timely submission of reports, and adherence to ethical guidelines and organizational policies.

This position focuses on clients experiencing major mental illnesses and severe emotional disorders. You will be working with diverse populations, including individuals with various cultural backgrounds and lived experiences. Recognizing and respecting these differences will be crucial in establishing rapport and eliciting positive outcomes for each client. Our hospice setting necessitates a sensitive understanding of the end-of-life process, allowing you to support clients' mental health while grieving.

• A Master's Degree in Social Work (MSW) or a related field is required.
• Current Licensed Clinical Social Worker (LCSW) licensure in the state of Illinois.
• Expert knowledge in assessment, diagnosis, and treatment methodologies pertinent to mental health.
• Strong verbal and written communication skills aimed at various audiences.
• Ability to work collaboratively and effectively across diverse teams of healthcare professionals.

While a minimum of 2 years of clinical experience is preferred, we are particularly interested in candidates who have demonstrated expertise in crisis management, risk assessment, and therapeutic interventions tailored for adult populations. Experience in hospice or end-of-life care settings will be considered a significant asset.

Pathway Counseling Services offers a competitive salary range of USD 70,000 to 85,000 annually, commensurate with experience and qualifications.
