Key Responsibilities
  • Manage a designated caseload of patients by conducting assessments to identify psychosocial factors affecting their health and well-being. Develop tailored treatment plans that incorporate various therapeutic modalities to support patients in overcoming barriers to care.
  • Lead multidisciplinary team discussions regarding complex psychosocial issues, life care decisions, and protective service concerns, actively contributing your expertise in creating effective solutions.
  • Engage with case managers and healthcare professionals to provide comprehensive education, resources, and referrals, while ensuring that psychosocial and socioeconomic needs are incorporated in treatment and discharge plans.
  • Supervise and mentor junior staff, including MSWs, interns, and volunteers, enhancing their skills and contributing to the department’s overall growth and service delivery standards.
  • Conduct timely crisis intervention and support services for high-risk patients and their families, effectively addressing their immediate needs and facilitating essential follow-up actions.
  • Coordinate care for pediatric patients requiring partnership with community agencies to ensure continued support beyond discharge, optimizing resource utilization.
  • Proactively manage documentation and follow all ethical and professional standards to maintain the integrity of patient services, ensuring that treatment timelines are adhered to without avoidable delays.
Patient Population / Client Focus

Patients may include those with chronic illnesses, mental health challenges, and socioeconomic difficulties. Qualifications
  • Must possess a valid Licensed Clinical Social Worker (LCSW) credential.
