This position is primarily remote, allowing for flexibility while you serve clients across Sacramento, California, and beyond. You will collaborate digitally with your peers and have access to all necessary tools and systems required to facilitate remote counseling. • Conduct thorough assessments and evaluations of clients to accurately determine their mental health needs.
• Develop, implement, and modify treatment plans in collaboration with clients, utilizing evidence-based practices.
• Execute mandatory reporting processes and uphold duty-to-warn obligations as required by state regulations.
• Regularly monitor and document clients' progress, adjusting treatment approaches as necessary based on their evolving needs.
• Work alongside interdisciplinary teams—including doctors, social workers, and other mental health professionals—to holistically address clients' challenges.
• Collect and analyze client outcome data to evaluate treatment effectiveness and promote best practice methodologies.
• Maintain accurate, secure, and up-to-date records and documentation in compliance with legal and ethical standards.
• Stay informed about current trends and research in mental health to continuously enhance therapeutic skills and knowledge.

The clients served by Breakthrough Mental Health Services come from a wide array of backgrounds, including individuals facing depression, anxiety, trauma, and other mental health disorders. The ideal candidate will demonstrate a keen understanding of the cultural and socioeconomic factors that contribute to mental health challenges within our diverse patient population. • Master’s degree in Professional Counseling or a closely related field from an accredited institution.
• Active licensure as a Licensed Professional Clinical Counselor (LPCC) in California.
• Minimum of three years of clinical experience in a mental health setting, providing services to diverse populations.
• Proven proficiency in cognitive-behavioral therapy (CBT) and other evidence-based therapeutic methodologies.
• Strong ability to conduct comprehensive diagnostic evaluations and devise individualized treatment plans.
• Excellent verbal and written communication skills, with a demonstrated ability to foster therapeutic relationships.

• Experience in community health settings or public health clinics is highly regarded.
• Familiarity with telehealth platforms and remote therapy practices.
• Experience with data analysis and outcomes measurement to track client progress effectively.
