• You will conduct initial assessments for clients experiencing symptoms of major mental illness and severe emotional disorders, utilizing evidence-based screening tools to determine appropriate intervention strategies.
• As a key team member, you will participate actively in case conferences and team meetings, collaborating with other healthcare professionals to develop integrated care plans that meet each client's unique needs.
• You'll provide individual and group therapy sessions, ensuring that treatment is tailored to clients' presenting issues and encouraging their active participation in their care plans.
• In addition to direct clinical work, you will be responsible for monitoring client progress, adjusting treatment plans as needed, and ensuring that all documentation, reports, and data submissions are completed in a timely manner.
• The role requires you to be responsive to crises, providing intervention as needed during program hours and being available for on-call coverage to ensure continuous support for clients.
• Identifying opportunities to enhance service delivery, you will contribute to quality improvement initiatives and participate in committees aimed at advancing agency goals.

Your work will predominantly focus on clients enrolled in hospice care, many of whom may present with co-occurring mental health and emotional challenges as they navigate complex end-of-life issues. Engaging with clients from diverse backgrounds, you will respect their values and beliefs, tailoring your approach to fit their specific cultural and emotional needs.

• A Bachelor’s Degree in Social Work (BSW) is required. A Master's Degree or equivalent is preferred for this role.
• Current licensing as a Clinical Social Worker (LCSW) in the state of Illinois is mandatory.
• At least two years of experience working in a clinical setting, particularly with hospice or end-of-life services, is essential.
• Strong communication skills and the ability to build rapport with clients and coworkers alike are key to success in this role.

While not required, candidates with previous experience in hospice care or palliative contexts are highly desirable. Familiarity with electronic health records (EHR) systems and evidence-based therapeutic methodologies will also be greatly beneficial.

The position offers a competitive salary range of USD 70,000 to 85,000 annually, commensurate with experience.
