The Licensed Clinical Social Worker (LCSW) at Keck Medical Center of USC plays an integral role in delivering comprehensive psychosocial support to patients and their families within our Community Health Clinic. Key Responsibilities
  • Psychosocial Assessment: Conduct thorough and timely psychosocial assessments that take into account human behavior and psychosocial factors influencing treatment outcomes. Recognize and address potential ethical or risk issues throughout the assessment process.
  • Clinical Intervention: Deliver supportive counseling to patients and families while skillfully linking them to necessary community resources and services. Facilitate family discussions to navigate sensitive issues like hospice care, DNR orders, and various placement options.
  • Case Management: Balance a diverse clinical caseload by identifying high-risk situations, managing referrals, and coordinating ongoing patient care. Consult with team members and supervisors as necessary to ensure all patient needs are met effectively.
  • Legal and Ethical Compliance: Maintain adherence to legal and ethical standards in all assessments and interventions, including actions regarding suspected abuse or neglect and patient rights.
  • Documentation: Complete necessary documentation as per organizational policies, including social services progress notes and interdisciplinary plans of care.
  • Team In-Service Training: Provide training and support to hospital staff on psychosocial patient issues through initiatives like Lunch and Learns.
  • Quality Improvement: Participate in developing, monitoring, and analyzing indicators that enhance patient care quality throughout the organization.
  • Addtional Duties: Engage in community outreach and support activities, including programs during Social Work Month and educational groups for patients and families.

Qualifications
