Key Responsibilities
  • Conduct thorough and ongoing assessments of clients’ mental health needs, tailoring your approach to facilitate engagement and progress.
  • Maintain clear and effective communication with supervisors, direct care staff, and family members regarding clients’ progress and treatment plans.
  • Develop and provide a tentative schedule of appointments, including individual sessions, home visits, and observations, to ensure comprehensive support for your clients.
  • Coordinate home visits for in-home observations, chart reviews, and staff interviews, employing analytical skills to review data and suggest improvements.
  • Implement and monitor therapeutic programs, ensuring they align with the goals of individual treatment plans and adjusting as necessary.
  • Participate actively in Interdisciplinary Team Meetings, contributing insights and updates on client progress and program efficacy.
  • Document monthly and quarterly progress notes detailing clients’ journeys and progress towards their therapeutic goals.
  • Organize and lead training sessions to certify staff on the implementation of therapeutic plans and documentation procedures.
  • Conduct follow-up interviews and home visits as required, ensuring ongoing support and adjustment of treatment strategies based on client feedback.
  • Maintain accurate and organized records to facilitate availability for current and future evaluation by relevant parties.
  • Act as a liaison with the mental health community, helping to connect clients with additional resources and support.
  • Assist in the creation of training materials and procedures aimed at enhancing program quality and implementation for client benefit.
Patient Population / Client Focus

In this role, you will engage with a diverse range of clients, including both adults and youth, addressing various mental health challenges, such as anxiety, depression, trauma, and relationship issues. Qualifications
