Remote Contingent Dietician-Trinity Health at Home Infusion

8-16 hrs a week

100% Remote but must be licensed in State of Iowa*

Basic Job Function Summary
The Dietitian is responsible for the provision of quality services that promote nutritional health and effective dietarypatterns for patients. Acts as a direct link between patients and MercyOne Des Moines HH and Hospice.


Required Education
Currently licensed to practice in the state serviced.
Registered by the American Dietetic Association.
Bachelor's degree in dietetics, foods, and nutrition; food service systems management; or related area.

Minimum Qualifications
Strong written and verbal skills.
Ability to work closely with a wide range of professionals or a multidisciplinary team.
Interpersonal skills that facilitate inter- and intra-team/departmental communication.
Excellent organizational and time management skills.
Excellent attention to detail.
One year experience in a hospital, nursing home, or home infusion pharmacy working with TPN and enteral
patients.

Essential Functions
1. Supports Trinity Health at Home's mission, values, strategic goals, and high standards of customer
service Consistently lives People First Behaviors.
2. Adheres to the Corporate Compliance Program, including confidentiality of HIPAA protected health
information.
3. Provides care following the plan of treatment and patient care plan.
4. Completes nutritional assessments for enteral and parenteral patients at admission and reassessments as
needed and documents assessments in electronic medical record.
5. Qualifies Enteral and Parenteral patients for insurance coverage, ensuring guidelines for coverage are
met and supporting documentation is collected.
6. Appropriately uses clinical notes, care plans, and other documentation tools.
7. Completes and submits all required documentation in a timely and accurate manner.
8. Conducts comprehensive patient assessments that include information regarding general health and
socioeconomic status.
9. Establishes a care plan for continued needs based on the patient assessment, information from caregivers
or other service providers relative to nutritional concerns and physician orders.
10. Works in conjunction with Clinical Pharmacist to make recommendations for changes to parental nutrition
formulas for patients based upon assessments, lab values and clinical need.

11. Provides education to patients and caregivers.
12. Utilizes and coordinates patient resources to meet nutritional requirements.
13. Demonstrates sensitivity to patient and caregiver needs, custom or feelings.
14. Attends and participates in case conferences, in-services, and continuing education programs.
15. Participates in Quality Assurance Performance Improvement (QAPI), including assisting with the overall
development and implementation of the PI plan, assisting in the identification of goals and related patient
outcomes, participation and reporting of activities and outcomes, development and implementation of
corrective plans.
16. All other duties as assigned.
Mental and Physical Requirements
Frequently lifts, stands, and walks (65% of time)
Occasionally bend, stoop, and twist (30% of time)
Occasionally position and lift patients (10% of time)
Ability to perform continuously detailed tasks with frequent interruptions and work reprioritization (90% of
time)
Must be able to adapt to frequently changing work parameters.
Must be able to travel to various TH/THHS worksites or various work locations as required.

with applicable standards and procedures relevant to the position's job duties.
