• Conduct comprehensive psychosocial assessments to evaluate the emotional, social, and environmental needs of patients and families.
• Develop and implement individualized treatment plans based on patient evaluations, which may involve therapy sessions, support group facilitation, and crisis intervention.
• Provide feedback and education to caregivers, enhancing their skills in responsive parenting and addressing their specific concerns regarding their children’s emotional and social needs.
• Maintain thorough documentation including progress notes, treatment outcome assessments, and review of medical reports as required by regulatory standards.
• Collaborate with healthcare professionals to provide insights into psychosocial issues and effectively connect patients with community resources for ongoing support.

In this role, you will primarily work with individuals facing serious health challenges and their families within a hospice setting. This includes providing support to terminally ill patients, assisting with grief counseling, and ensuring that emotional burdens are addressed holistically. As an LICSW, you will play a vital role in enhancing the quality of life for patients through empathetic and individualized care, facilitating a smoother transition during difficult times.

• Master’s degree in Social Work (MSW) or a related field along with a valid LICSW license in Washington State.
• A minimum of 2 years of progressive experience in social work, particularly in psychiatric settings, highlighting expertise in service delivery and patient management.
• Demonstrated cultural competency and the ability to work effectively with diverse populations across various life stages, from children to the elderly.
• Current certification in the safe management of disruptive and assaultive behavior is preferred.

• Familiarity with community resources and programs that support health and wellness, particularly for families dealing with chronic illness or grief.
