 What You’ll Do:
 
  • Conduct comprehensive in-home client assessments and develop individualized Plans of Care. 
  • Perform supervisory and quality assurance visits to ensure exceptional client care and regulatory compliance. 
  • Educate clients, families, and caregivers regarding health conditions, medications, nutrition, and wellness. 
  • Collaborate with physicians, referral partners, and the interdisciplinary care team to coordinate client services. 
  • Monitor client status, recognize changes in condition, and communicate recommendations to the appropriate healthcare providers. 
  • Mentor and support caregivers through clinical guidance, education, and competency validation. 
  • Maintain timely, accurate, and compliant electronic documentation. 
  • Participate in quality improvement initiatives while helping support the continued growth of our clinical services
 Are You the Right Fit?
 
  • Current, unrestricted Registered Nurse license in the State of New Jersey. 
  • Minimum of five (5) years of professional nursing experience required. 
  • A minimum of three (3) years of recent home care or hospice nursing experience is strongly preferred. 
  • Demonstrated ability to perform independent clinical assessments, develop individualized Plans of Care, and make sound clinical decisions in the home setting. 
  • Strong knowledge of geriatric care, chronic disease management, and home care best practices. 
  • Demonstrated ability to work independently, prioritize competing demands, and exercise sound clinical judgment in the home setting.
  • Excellent communication and relationship-building skills. 
  • Proficiency with electronic health records and technology. 
  • Current CPR certification. 
  • Knowledge of New Jersey home care regulations preferred. 
  • Valid driver's license, reliable transportation, and willingness to travel throughout Atlantic and Cape May Counties.
