• Conduct thorough clinical assessments through telehealth platforms, developing personalized treatment plans informed by best practices and patient narratives.
• Deliver various therapeutic interventions, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), tailored to the psychological and emotional needs of hospice patients.
• Maintain meticulous and timely clinical records utilizing secure electronic health record (EHR) systems, ensuring compliance with federal regulations and organizational standards.
• Collaborate with interdisciplinary teams of healthcare professionals, including physicians, psychologists, and nurses, through virtual meetings to coordinate comprehensive care.
• Engage in ongoing professional development via webinars, virtual workshops, and case consultations to remain at the forefront of mental health practices.
• Educate patients and their families about mental health issues, available treatment options, and community resources that can provide additional support.
• Manage a caseload of remote clients effectively, ensuring timely achievement of treatment goals while adhering to the ethical codes established by the National Association of Social Workers (NASW).

• A Master's degree in Social Work from an accredited institution is required.
• A valid license to practice as a Licensed Clinical Social Worker (LCSW) in the state of Connecticut.
