• Conduct comprehensive mental health evaluations that take into account existing medical conditions impacting mental well-being.
• Design and implement personalized treatment plans that address both psychiatric and medical issues, ensuring integrated solutions for patients.
• Facilitate individual and group therapy sessions using evidence-based methodologies tailored to meet each patient’s specific needs.
• Engage in collaborative treatment planning with medical professionals in our network to enhance coordinated care delivery.
• Provide immediate support and crisis intervention during mental health emergencies, utilizing appropriate de-escalation techniques.
• Educate patients on the interplay between mental and physical health, empowering them to take charge of their overall wellness.
• Maintain meticulous and confidential documentation in adherence to ethical and legal standards, utilizing electronic health records systems.
• Contribute to team meetings and case conferences, sharing insights and updates on patient progress to enhance interdisciplinary collaboration.
• Stay informed about ongoing research and developments in integrated care, continually integrating best practices into your work.

In this role, you will primarily work with patients who present a unique blend of mental health challenges alongside complex medical conditions. These may include, but are not limited to, those suffering from chronic pain, symptoms related to autoimmune diseases, or substance use disorders. • A Master's or Doctoral degree in Social Work, Mental Health Counseling, Psychology, or Marriage and Family Therapy from an accredited institution.
• Active and valid licensure as an LCSW, LPCC, or LMFT in California.
• A minimum of two years of experience in mental health counseling, particularly in environments catering to individuals with co-occurring medical conditions.
• Exceptional interpersonal and communication skills, enabling you to build trust and rapport with patients effortlessly.
• Strong organizational capabilities with a commitment to maintaining professional boundaries and confidentiality.
• Proficient in using electronic health record systems and standard computer applications.

• Experience working within a telehealth setting is advantageous but not mandatory.
• Familiarity with evidence-based therapeutic approaches such as Cognitive Behavioral Therapy (CBT) or Dialectical Behavior Therapy (DBT).
• Proven ability to work collaboratively in an interdisciplinary team setting, promoting comprehensive patient care.
