• Coordinate and document transitions for clients transferring from hospital or inpatient services, ensuring strong communication and collaboration with hospital staff for safe and effective discharges.
• Utilize crisis de-escalation techniques to support individuals in distress, providing direct milieu support when acting as the Clinician of the Day.
• Conduct thorough risk assessments and implement safety plans, documenting these processes on the same day for all clients at increased risk of crisis interventions.
• File critical incident reports within 24 hours when necessary and manage after-hours care coordination support on an alternating schedule.
• Proactively collaborate with clients to prevent the necessity of crisis services, maintaining crisis alerts for coordination with county healthcare resources.
• Complete various administrative duties efficiently while ensuring high-quality service delivery in a busy caseload environment.

This role emphasizes working with a diverse population of individuals facing mental health challenges, including anxiety, depression, trauma, and substance use disorders. Our practitioners are encouraged to build therapeutic relationships, fostering trust and empowerment to enhance clients' self-management skills and overall resilience.

• A master's or doctorate degree in clinical psychology, counseling, clinical social work (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD) from an accredited institution.
• A valid independent license (LCSW, LMFT, LPC) in the state of residence, allowing for the provision of therapy and counseling services.
• Strong communication and interpersonal skills, with the ability to maintain a calm and empathetic demeanor in crisis situations.
• Proven ability to manage a high caseload while maintaining quality of care through effective triage and delegation of tasks.

• Demonstrable experience in crisis intervention, mental health counseling, and care coordination, preferably in telehealth settings.
• Strong familiarity with electronic health records (EHR) and teletherapy platforms.
• Experience in developing crisis management plans and working within interdisciplinary teams to optimize patient outcomes.
