Work Environment

This role offers a completely remote work setting, allowing you to manage your schedule while providing vital services to seniors. With a full-time work schedule from Monday to Friday, 8 AM to 4:30 PM, you will have the opportunity to adapt your hours to meet client needs, which may include occasional evening or weekend consultations. Your remote position ensures you can directly engage with clients through virtual platforms, thus increasing access to care for those who might face mobility challenges.

Key Responsibilities
  • Conduct comprehensive assessments of clients’ psychosocial needs to identify emotional, mental, and social factors affecting their well-being.
  • Create individualized care plans in collaboration with clients and their families, aimed at achieving specific health and lifestyle goals.
  • Provide psychoeducation, counseling, and emotional support to clients and their families to help them navigate transitions, stressors, and life challenges.
  • Advocate for clients' rights, ensuring timely access to community resources and necessary services to optimize holistic care.
  • Work closely with a multidisciplinary team, including physicians, nurses, and caregivers, engaging in case discussions to coordinate seamless care delivery.
  • Perform routine follow-ups to monitor clients’ conditions, evaluate their living situations, and address any emerging safety or health concerns.
  • Facilitate dialogue and conflict resolution among family members relating to client care to promote a collaborative approach in support systems.
  • Educate clients and their families on available community resources, social activities, and support groups, aiming to enhance their quality of life.
