A Remote Community Health Worker (CHW) is a trained professional who provides support, education, and resources to individuals and communities to improve health outcomes. Unlike traditional CHWs, remote CHWs work virtually via phone, video calls, or online platforms to connect with patients, provide health coaching, coordinate care, and assist with social services. They often serve as a bridge between healthcare providers and underserved populations, helping to address barriers to care. A typical day for a Remote Community Health Worker involves conducting outreach and follow-ups with clients via phone, video calls, or messaging platforms to assess health needs and provide education or resources.
