You may spend time coordinating care by connecting clients to local services, maintaining detailed case notes in online systems, and collaborating with healthcare providers or social service agencies. While much of the work is self-directed, regular virtual team meetings ensure coordination and support. The flexibility of remote work allows you to manage your schedule while still making a meaningful impact in communities. This role requires effective time management and initiative to ensure all clients receive the support they need.
