• Provide remote social work services that include individualized assessments, treatment planning, and direct client intervention.
• Conduct thorough biopsychosocial assessments to identify client needs, strengths, and vulnerabilities, ensuring that treatment plans are tailored to each individual.
• Utilize evidence-based practices to develop and implement effective treatment strategies, including therapy, counseling, and resource coordination.
• Advocate effectively for clients' rights and access to critical resources such as housing, healthcare, and community services.
• Work collaboratively with a diverse range of professionals including psychologists, vocational counselors, and healthcare providers to ensure holistic care for clients.
• Engage in case management duties, tracking client progress, adjusting treatment plans as necessary, and documenting outcomes diligently in electronic health records.
• Participate in regular team meetings, providing insights and learning from peers to enhance service delivery and client outcomes.

The ideal candidate will be focused on serving a diverse client base, including individuals and families facing various mental health challenges, socioeconomic barriers, and cultural considerations. • A Master’s degree in Social Work (MSW) from an accredited institution.
• Active Licensed Clinical Social Worker (LCSW) license in the state of Oregon.
• At least 3 years of experience in social work, with a strong background in community health practices preferred.
• Exceptional communication skills that enable you to connect empathetically with clients in a remote setting.
• Proficiency with telehealth technologies and virtual communication platforms.

• Experience working in a community health clinic or similar setting.
• Familiarity with trauma-informed care practices.
• Background in working with underrepresented populations and understanding the social determinants of health.
