• Conduct detailed assessments and screenings aimed at understanding individual patient mental health needs, which will inform tailored treatment plans.
• Provide one-on-one counseling sessions and crisis intervention services for patients experiencing a wide range of psychological and emotional challenges.
• Develop and facilitate therapy groups and educational workshops that promote essential skills for mental well-being and personal development.
• Actively collaborate with interdepartmental teams, ensuring coordination with medical professionals, community organizations, and patient support services to deliver comprehensive care for our patients.
• Participate in outreach initiatives that enhance mental health awareness and promote wellness in the local community.
• Maintain meticulous and confidential patient records, adhering to ethical guidelines and legal requirements.

This position primarily engages with a diverse patient population within the community health system in Modesto. The role will demand a deep understanding of the mental health issues common among various demographics, including adults and adolescents coping with stress, anxiety, depression, trauma, and substance use disorders.

• An active and valid state license as a: Licensed Clinical Social Worker (LCSW), Licensed Professional Clinical Counselor (LPCC), or Licensed Marriage and Family Therapist (LMFT).
• A Master’s degree, or higher, in Counseling, Psychology, Social Work, or a related field.
• A strong grasp of the unique psychological challenges faced by patients in a community health setting.
• Excellent verbal and written communication skills, fostering effective interactions with patients and collaboration with multidisciplinary teams.
• Proficiency with evidence-based practices and clinical techniques pertinent to conducting therapy in a community health environment.
• A demonstrated ability to manage a diverse caseload in a high-volume setting effectively, prioritizing clinical needs efficiently.

• Previous experience in providing mental health services in community health or hospital settings.
• Familiarity with Telehealth platforms and remote communication tools, ensuring seamless interaction with clients.
• Experience in leading group therapy sessions or workshops that focus on mental health skills development.

• Annual salary ranging from $115,000 to $130,000, commensurate with experience and qualifications.
