• Conduct in-depth psychosocial assessments to understand the unique needs of each patient and their families, identifying mental health challenges and strengths.
• Deliver individualized therapy sessions via telehealth platforms, utilizing evidence-based therapeutic modalities such as cognitive-behavioral therapy (CBT) and mindfulness techniques.
• Collaborate with interdisciplinary teams, including physicians and nurses, to formulate and implement personalized treatment plans focused on enhancing patient quality of life.
• Facilitate group therapy sessions and psychoeducation classes for patients and family caregivers, providing valuable support and resources.
• Develop crisis intervention plans, conduct risk assessments, and provide safety planning in high-stress situations.
• Maintain meticulous and timely documentation of client interactions, assessments, and treatment plans, ensuring compliance with ethical and legal standards.
• Remain apprised of the latest research and developments in social work methodologies to enhance therapeutic approaches and patient outcomes.

You will be working with a diverse patient population, including individuals facing terminal illnesses and their families navigating complex emotional landscapes. Your role is integral in providing grief counseling and psychosocial support, helping clients achieve a better quality of life and fostering resilient coping strategies during challenging times.

• Master's degree in Social Work from an accredited institution.
• Active and unrestricted licensure as a Licensed Clinical Social Worker (LCSW) in the state of Georgia.
• Minimum of two years of experience working in a mental health or outpatient setting, with expertise in hospice or palliative care preferred.
• Comprehensive knowledge of therapeutic interventions and evidence-based treatment frameworks.
