This role is primarily remote, providing flexibility to engage with clients from various locations in Clay, New York, and surrounding areas. • Provide direct clinical support to clients coping with major mental illnesses and emotional disorders, both in the home and through virtual sessions.
• Engage in collaborative team efforts to address common goals, demonstrating strong teamwork and communication skills.
• Conduct thorough assessments and screenings to determine individual needs, while ensuring all interventions are evidence-based and culturally appropriate.
• Create and implement integrated care plans, facilitating coordinated efforts among clients, families, and healthcare providers to meet complex needs.
• Offer a range of therapeutic services, including individual, group, and family therapy, to foster recovery and improve patient well-being.
• Actively monitor clients' progress, managing caseloads effectively to ensure timely access to treatment and continuity of care.
• Participate in crisis intervention efforts and provide on-call support as needed to manage urgent situations.
• Ensure compliance with all ethical guidelines and uphold the integrity of Bayfront’s code of conduct.
• Contribute to quality improvement initiatives by participating in committee work dedicated to enhancing service delivery.

• A Master's degree in Social Work (MSW) from an accredited program is required.
• Active licensure as a Clinical Social Worker (LCSW) in New York.

• Excellent verbal and written communication skills to articulate client needs and progress effectively.
• Demonstrated ability to work collaboratively in a team setting, respecting diverse traditions, beliefs, and interpersonal styles.

• A minimum of 2 years of clinical experience, ideally in hospice, palliative care, or related fields.
• Experience working with diverse populations and understanding cultural nuances affecting healthcare access and treatment.
• Background in geriatric care or mental health services related to end-of-life issues is highly desirable.

The position offers a competitive salary range of USD 70,000 to 90,000 annually, commensurate with experience and qualifications.
