• Conduct thorough bio-psychosocial evaluations to identify barriers to treatment and develop effective strategies for optimizing patient health outcomes.
• Collaborate with a multidisciplinary team to design and implement individualized treatment plans that utilize both individual and group therapy modalities.
• Document all social work activities and patient interactions systematically in the medical records, in accordance with departmental policies and state regulations.
• Provide ongoing consultation to medical and support staff regarding patients’ psychosocial needs to ensure an effective and integrated care approach.
• Educate patients and their families about medical and social work recommendations to facilitate understanding and encourage compliance with treatment protocols.
• Assess advocacy needs within the patient and family context, coordinating community resources and support systems to enhance patient stability and overall well-being.
• Maintain up-to-date knowledge of available social support resources to effectively address the needs of diverse patient populations.
• Engage in continuing education and professional development activities to enhance clinical competencies and stay informed about best practices in social work.
• Regularly produce case-related statistics and documentation as required by department standards.

As a Clinical Social Worker at Jimenez and Associates, you will primarily work with individuals facing a range of psychosocial issues, including mental health disorders and the effects of physical illness on mental well-being. You will support diverse patient populations with varying cultural backgrounds, addressing their unique needs through personalized care strategies designed to promote recovery and resilience.

• Master's Degree in Social Work from an accredited institution.
• A valid and current State License as a Licensed Clinical Social Worker (LCSW).
• Comprehensive knowledge of the NASW Code of Ethics and social work standards of practice, ensuring adherence to best practices in all professional interactions.

Candidates should possess between six months to one year of clinical experience, preferably within a health or mental health setting. Experience working with similar populations and the specific challenges they face will be beneficial.
