• Conduct thorough patient intakes, including diagnostic and functional assessments, and provide actionable recommendations to primary care providers for treatment planning.
• Support primary care providers by evaluating mental health and psychosocial issues, ensuring patients receive integrated healthcare tailored to their needs.
• Deliver brief, focused interventions to patients requiring immediate mental health support, utilizing best practices in therapeutic techniques.
• Develop and implement relapse prevention plans for at-risk patients, promoting long-term stability and resilience.
• Maintain detailed documentation of patient progress and treatment interactions, adhering strictly to agency and Federally Qualified Health Center (FQHC) guidelines.
• Coordinate mental health services, facilitating connections between patients, primary care providers, and external treatment facilities as appropriate and with patient consent.
• Track and monitor patient outcomes, adjusting treatment plans as necessary to ensure optimal therapeutic effectiveness.
• Participate in interdisciplinary team meetings to enhance patient care and contribute insights on behavioral health services.
• Advocate for patient self-management skills through educational resources, empowering them to take an active role in their recovery journey.
• Maintain compliance with the Code of Ethical Conduct for Clinical Professionals and uphold the integrity of our practice standards.
• Complete additional tasks as assigned, contributing to overall clinic operations and patient care initiatives.

As a Licensed Professional Counselor at Thrive Hard, you will encounter a diverse patient population whose needs span various mental health and psychosocial challenges. This role requires a nuanced understanding of the complexities individuals face, ranging from anxiety and depression to broader community-related stressors. Your ability to connect with clients and provide culturally competent care will directly impact their healing and integration within the community.

• Master’s Degree in Clinical Counseling or related field.
• Valid LPC (Licensed Professional Counselor) License in Texas.
• Comprehensive knowledge of mental health assessments, treatment methodologies, developmental psychopathology, and psychotherapeutic techniques.
• Understanding of various psychogenic issues and their effects on families and communities.
• Strong communication and interpersonal skills to effectively engage with patients and collaborate with a multidisciplinary team.
• Proficiency in electronic health record systems for documentation and patient management.
• Ability to work independently in a remote environment while maintaining productivity and service quality.

• Experience in providing telehealth services is a plus.
• Fluency in both English and Spanish is preferred but not required, enhancing our ability to serve multilingual clients.
• Prior experience in a primary care or integrated behavioral health setting.

The salary range for this position is competitive, ranging from USD 87,000 to 105,000 annually, depending on experience and qualifications.
