As a member of our hospice center, you will have the opportunity to work alongside a diverse team of experienced professionals, including nurses, physicians, and other healthcare providers, as you deliver empathetic support to patients facing end-of-life challenges. • Provide timely and respectful therapy sessions to clients according to their scheduled appointments, ensuring adherence to the individual care plans and goals.
• Maintain comprehensive documentation of client interactions, ensuring that all reports are completed within 72 hours of service delivery and comply with California Board of Behavioral Sciences regulations.
• Foster collaborative relationships with support staff to manage a full schedule of appointments and streamline the therapeutic process.
• Conduct psychosocial assessments, including detailed interviews and evaluations, addressing factors such as mental health crises, family dynamics, and resource access.
• Actively uphold ethical standards in all client interactions, aiming to create a therapeutic environment built on trust, respect, and professional integrity.

In this role, you will primarily work with patients in hospice care, which involves providing support for individuals grappling with terminal illnesses and their families. You will address a broad spectrum of psychosocial needs, from facilitating end-of-life conversations to helping clients and families navigate grief and loss. Your input will play a critical role in enhancing the quality of life for patients during this sensitive time.

• A Master's Degree in Social Work from an accredited institution, showcasing your commitment to professional excellence.
• A valid and unrestricted CA Clinical Social Worker License (LCSW) is a must for this position.
• Minimum of 2 years of direct clinical experience in a behavioral health setting, demonstrating your capacity to handle complex emotional and social challenges.
• Strong experience in case management, including taking psychosocial histories and executing effective treatment plans.
• Proficient conflict management skills, essential for navigating the challenging dynamics often present in hospice settings.

• Experience working specifically within hospice, palliative care, or end-of-life services, allowing for a deeper understanding of the unique needs of this patient demographic.
• Familiarity with electronic health record (EHR) systems and other telehealth platforms, ensuring seamless delivery of care and documentation.
• Demonstrated ability to work collaboratively within a multidisciplinary team to enhance patient care and outcomes.

The annual salary for this position ranges between $120,000 and $130,000, commensurate with experience and qualifications.
