Job description

Amergis Healthcare Staffing is seeking qualified LVN/LPNs to support a rapidly growing virtual healthcare organization specializing in preventive care, wellness services, chronic condition management, and patient support programs. In this fully remote role, LVNs work closely with providers and multidisciplinary healthcare teams to support care coordination, manage clinical documentation, review patient information, and assist with telehealth workflows that enhance the overall patient experience.

This opportunity is ideal for detail-oriented nursing professionals who thrive in a technology-driven environment and are comfortable supporting patients and providers through electronic health records, patient portals, and digital healthcare platforms. Candidates must be able to work independently, navigate multiple systems efficiently, and maintain the highest standards of patient confidentiality, clinical accuracy, and customer service while supporting a virtual care model.

Key Responsibilities
  • Review and maintain electronic patient records for accuracy and completeness
  • Manage and update clinical documentation within EHR/EMR systems
  • Review patient communications and escalate clinical concerns when appropriate
  • Coordinate laboratory orders, results, and follow-up documentation
  • Support medication refill requests and provider-directed care coordination activities
  • Collaborate with providers to ensure timely completion of patient care plans
  • Monitor patient-reported information and identify potential concerns for provider review
  • Maintain compliance with HIPAA and organizational privacy standards
  • Communicate professionally with patients, providers, pharmacies, and ancillary services
  • Participate in quality assurance, workflow improvement, and documentation compliance initiatives
Qualifications
  • Active LVN/LPN license required (Compact License preferred where applicable)
  • Minimum four (4) years of LVN/LPN experience required
  • Prior experience in telehealth, remote healthcare, care coordination, case management, or clinical documentation required
  • Experience with chart review, clinical documentation, and EHR/EMR systems
  • Strong critical thinking, clinical judgment, and problem-solving skills
  • Excellent written and verbal communication abilities
  • Ability to manage multiple priorities in a fast-paced virtual environment
Required Skills (Non-Negotiable)
  • Prior remote or telehealth experience
  • Strong medication knowledge, dosing, and calculation skills
  • High level of comfort navigating EHRs, patient portals, and multiple digital platforms
  • Excellent technical proficiency and computer skills
  • Ability to work independently with minimal supervision
  • Strong documentation accuracy and attention to detail
  • Reliable high-speed internet connection
  • Excellent organizational, time-management, and problem-solving skills
  • Ability to maintain patient confidentiality and HIPAA compliance while working remotely
  • Availability to support assigned schedules, including evenings, overnight shifts, and rotating weekends as required by business needs

Work Setting: Fully Remote
Assignment Type: Full-Time, Temp-to-Hire
Schedule: 40 hours/week with rotating weekend coverage as required.
